Génotypage de l’UGT1A1*28 et dosage plasmatique de l’irinotécan et son métabolite actif (SN38) par HPLC-MS/MS en vue d’une adaptation posologique et prévention de sa toxicité dans le traitement du cancer colorectal métastatique

dc.descriptionIntroduction La variabilité interindividuelle de la pharmacocinétique de l’irinotécan (CPT-11) et de son métabolite actif SN-38 influence significativement la survenue de toxicités chez les patients atteints de cancer colorectal métastatique. Le polymorphisme UGT1A1*28 et la bilirubine totale sont des facteurs modulant la clairance du SN-38 et le risque de toxicité. Aucune étude algérienne n’a évalué l’association entre ce génotype et l’exposition plasmatique à l’irinotécan et au SN-38. Méthodes Une étude prospective a été menée chez 83 patients atteints de cancer colorectal. Le génotypage UGT1A1*28 a été réalisé par qPCR. Les concentrations plasmatiques de CPT-11 et SN-38 ont été mesurées à 2 et 48 heures après perfusion par HPLC-MS/MS. Les corrélations entre le génotype, la bilirubine, l’exposition aux métabolites et les principales toxicités cliniques (diarrhée, neutropénie, nausées/vomissements) ont été analysées. Résultats Les fréquences génotypiques observées étaient de 38,6 % pour *1/*1, 49,4 % pour *1/*28 et 12 % pour *28/*28, conformes aux profils méditerranéens et nord-africains. Le génotype *28/*28 n’a pas influencé les concentrations de CPT-11, mais a entraîné une exposition significativement accrue et prolongée au SN-38. La bilirubine totale pré-traitement s’est corrélée positivement avec l’exposition tardive au SN-38. Le risque de diarrhée sévère était plus élevé chez les patients homozygotes *28/*28, tandis que la neutropénie et les nausées/vomissements montraient des associations plus limitées. Conclusion L’exposition au SN-38, plutôt qu’au CPT-11, constitue le principal déterminant pharmacocinétique de la toxicité. L’évaluation du génotype UGT1A1*28 et de la bilirubine, combinée au suivi pharmacocinétique du SN-38, permet d’adapter la posologie et de réduire la toxicité

The UGT1A1*28 polymorphism and total bilirubin are factors that modulate SN-38 clearance and the risk of toxicity. No Algerian study has evaluated the association between this genotype and plasma exposure to irinotecan and SN-38. Methods A prospective study was conducted in 83 patients with colorectal cancer. UGT1A1*28 genotyping was performed by qPCR. Plasma concentrations of CPT-11 and SN-38 were measured at 2 and 48 hours after infusion using HPLC-MS/MS. Correlations between genotype, bilirubin, metabolite exposure, and major clinical toxicities (diarrhea, neutropenia, nausea/vomiting) were analyzed. Results The observed genotype frequencies were 38.6 % for *1/*1, 49.4 % for *1/*28, and 12 % for *28/*28, consistent with Mediterranean and North African profiles. The *28/*28 genotype did not affect CPT-11 concentrations but was associated with significantly increased and prolonged exposure to SN-38. Pre-treatment total bilirubin correlated positively with late SN-38 exposure. The risk of severe diarrhea was higher in homozygous *28/*28 patients, whereas associations with neutropenia and nausea/vomiting were more limited..
