| dc.description.abstract | Tuberculosis (TB) remains a major public health problem worldwide and particularly in Algeria, a country with intermediate endemicity. Latent tuberculosis infection (LTBI) represents a significant risk of reactivation in patients with chronic inflammatory diseases on anti-TNF therapy due to the key role of TNF-α in maintaining granulomas. The main objective of this study is to evaluate the QuantiFERON-TB Gold Plus (QFT-Plus) test for screening LTBI before anti-TNF treatment in patients with chronic inflammatory diseases at Sétif University Hospital. This retrospective observational study included 117 patients. All underwent pre-therapeutic assessment with clinical exam, chest X-ray, and QFT-Plus (with or without IDR). QFT-Plus was positive in 9 patients (7.7%). Concordance with IDR was very good and indeterminate results were very low (<1%). Anti-TB prophylaxis was started in all positive cases. These results highlight QFT-Plus as a reliable and practical tool, especially in BCG-vaccinated individuals, and confirm the importance of systematic screening before anti-TNF therapy in Algeria | |
| dc.description.sponsorship | La tuberculose (TB) demeure un problème majeur de santé publique dans le monde et particulièrement en Algérie, pays à endémie intermédiaire. L’infection tuberculeuse latente (ITL) pose un risque important de réactivation chez les patients sous biothérapie anti-TNF en raison du rôle clé du TNF-α dans le maintien des granulomes. L’objectif principal de cette étude est d’évaluer l’utilisation du QuantiFERON-TB Gold Plus (QFT-Plus) pour le dépistage de l’ITL avant traitement anti-TNF chez les patients atteints de maladies inflammatoires chroniques au CHU de Sétif. Il s’agit d’une étude observationnelle rétrospective sur 117 patients. Tous ont bénéficié d’un bilan pré thérapeutique incluant clinique, radiographie thoracique et QFT-Plus (± IDR). Le QFT-Plus était positif chez 9 patients (7,7 %). La concordance avec l’IDR était très bonne et le taux de résultats indéterminés très faible (<1 %). Une prophylaxie antituberculeuse a été instaurée chez tous les positifs. Ces résultats soulignent l’intérêt du QFT-Plus comme outil fiable et pratique, surtout chez les vaccinés BCG, et confirment l’importance du dépistage systématique avant biothérapie anti-TNF en Algérie. | |
