Abstract
The first permanent molars a symbolic tooth in odontology, it plays a key role in the growth and development of dental arches and occlusal functions. Due to its particular anatomy at both coronal and root level; its position in the arch and subsequent relationships, contributes to the child's maxillomandibular growth in the three dimensions of space, the protection of the temporomandibular joints and the stability of the occlusion. In the case of young people, the first permanent molar remains the most affected tooth by the carious disease due to its complex occlusal morphology, its posterior position on the arch; the immaturity of its dental tissues and the early emergence, when the Oral hygiene is still in growth for children.
Another attack also affects this tooth with a growing prevalence known as: Hypomineralization of the Incisors and Molars (MIH), in degrees ranging from mild to moderate and severe. Faced with a major decay of this tooth, the practitioner is faced with difficult and important choices for the child: extracting, delaying or keeping, and another difficulty that is added is to understand the behavior of the young patient and know how to act on different therapies
