Etude des marqueurs immunologiques et genetiques de la cholangite biliaire primitive dans la region de SETIF

dc.description.abstractPrimary biliary cholangitis (PBC) is a chronic autoimmune liver disease characterized by progressive destruction of the small intrahepatic bile ducts. Without treatment, it can progress to cirrhosis and end-stage liver failure. In Algeria, data on the immunological and genetic profiles of PBC patients remain scarce. The main objective of this study was to characterize the immunological profiles of PBC patients in the Sétif region and to assess their clinical correlations. Secondary objectives included the evaluation of selected genetic polymorphisms (SNPs) and the detection of novel specific autoantibodies. This was a case-control study conducted from March 2022 to November 2024, including fifty-six patients fulfilling the EASL (2018) diagnostic criteria for PBC and fifty-six matched healthy controls. The analysis involved detection of autoantibodies (ANA, AMA-M2, SP100, GP210, PML, anti-HK1, anti-KLHL12), immunoglobulin levels, complement fractions, specific serum proteins, and five genetic polymorphisms analyzed by real-time PCR (TaqMan® technology). AMA-M2 was detected in 80.4% of patients. The addition of anti-SP100 and anti-GP210 raised the diagnostic sensitivity to 92.9%, significantly reducing cases. A predominant elevation of IgG was observed in the majority of patients. Anti-HK1 antibodies were present in 36% of cases and were absent in healthy controls. A significant association was found between the rs5029939 polymorphism of the TNFAIP3 gene and advanced liver complications, while the other SNPs (IRF5, IRF7, STAT4, CD247) showed no significant associations. PBC patients in the Sétif region display an immunological profile largely comparable to that reported in Western countries, with notable particularities, especially the predominant IgG elevation. These findings highlight the value of an integrated diagnostic approach combining serological markers and genetic analyses to better identify and understand severe disease forms.

En l’absence de traitement, elle peut évoluer vers une cirrhose et une insuffisance hépatique terminale. En Algérie, les données spécifiques à cette pathologie restent limitées, notamment en ce qui concerne les profils immunologiques et génétiques des patients. L’objectif principal de cette étude est de caractériser le profil immunologique des patients atteints de CBP dans la région de Sétif, et d’étudier leur association avec les caractéristiques cliniques. Les objectifs secondaires visent à évaluer certains marqueurs génétiques (SNP) et la recherche de nouveaux auto-anticorps spécifiques. Il s’agit d’une étude cas-témoins menée entre mars 2022 et novembre 2024. Cinquante-six patients répondant aux critères diagnostiques de l’EASL (2018) et cinquante-six témoins sains appariés ont été inclus. Les investigations ont porté sur le dosage des auto-anticorps (AAN, AAM-M2, SP100, GP210, anti-HK1, anti-KLHL12), des immunoglobulines sériques, des fractions du complément, des protéines sériques spécifiques ainsi que sur l’analyse de cinq polymorphismes génétiques par PCR en temps réel (TaqMan®). L’AAM-M2 a été détecté chez 80,4 % des patients. L’ajout des anti-SP100 et anti-GP210 a permis d’augmenter la sensibilité diagnostique à 92,9 %, réduisant significativement le nombre de formes séronégatives. Une élévation de l’IgG a été observée chez la majorité des patients. Les anticorps anti-HK1 ont été retrouvés dans 36 % des cas, avec une spécificité marquée vis- à-vis des témoins. L’analyse génétique a mis en évidence une association significative entre le polymorphisme rs5029939 du gène TNFAIP3 et la présence de complications hépatiques avancées, tandis que les autres SNP (IRF5, IRF7, STAT4, CD247) n’ont pas montré de lien statistique significatif. La CBP dans la région de Sétif présente un profil immunologique comparable à celui des pays occidentaux, avec des particularités, notamment l’élévation prédominante des IgG. Ces résultats soulignent l’intérêt d’une approche combinant marqueurs sérologiques et analyses génétiques pour mieux identifier et comprendre les formes sévères.
