| dc.description | الملف الطبً للمرٌض هو سند مخصص لجمع كامل المعلومات الطبٌة للمرٌض سواء كان على ورق أو على نموذج إلكترونً فإنه ٌشكل عنصر أساسً فً نوعٌة و جودة العلاج الطبً بما أنه ٌشكل أداة متابعة لكامل التدابٌر الطبٌة ضمان أمن و سٌرورة العلاج الحسنة و كذا الجانب الاقتصادي و تحسٌن التواصل بٌن أعضاء الطاقم الطبً و تقٌٌم أدائه و هذا فً إطار برنامج التنمٌة المستدامة بٌن الطبٌب و المرٌض فً الجزائر الملف الطبً للمرٌض لا ٌخضع لأي تعلٌمة أو مرسوم صرٌح و كذلك هو الحال فً المركز الاستشفائً الجامعً و كنتٌجة لذلك بقً حال الملف الطبً غٌر واضح و متعلقا بإرادة الطبٌب و خبرته فً مٌدان العمل تعرف رقمنته بداٌتها و لكن دمج هذا الأخٌر ٌعد جد ضئٌل على مستوى المراكز الصحٌة أما بالنسبة للمركز الاستشفائً الجامعً سطٌف ٌعد هذا العمل فً طرٌق التجسٌد الملف الطبً للمرٌض التنمٌة المستدامة نوعٌة و جودة العلاج الطبً رقمنة الملف الطبً | |
| dc.description.abstract | The Medical Record of the Patient is a document gathering all the information concerning the person treated. Whether it is on paper or in the form of computers, it may be an imperative for the quality of care, since it is a tool for the traceability of medical procedures, security of care, continuity of care, Medico-economic cooperation, coordination and communication between professionals and also the evaluation of professional practices. That said, this holds as much as possible the doctorpatient relationship. In Algeria, the medical file of the patient is not the subject of a clear regulation or a strict recommendation as is the example of the CHU in Setif. As a result, his dress remains vague and reduced to the individual will of the doctors as well as their experience in the work. Its computerization has only just begun and its implementation is proving to be a snippet in a few healthcare facilities. In the case of the CHU in Setif, computerization of the patient's medical record is underway. | |

Sa tenue, qu’il soit sur support papier ou informatique représente sans doute un impératif de la qualité des soins puisqu’il constitue un outil au service de la traçabilité des actes médicaux, la sécurité des soins, la continuité des soins, l’aspect médico- économique, la coordination et la communication entre les professionnels et aussi l’évaluation des pratiques professionnelles. Ceci dit que cette tenue favorise au maximum la relation médecin-patient. En Algérie, le Dossier Médical du Patient ne fait pas l’objet d’une réglementation claire ni d’une recommandation stricte comme l’est l’exemple du CHU Sétif. Par conséquent, sa tenue reste vague et réduite à la volonté individuelle des médecins ainsi qu’à leur expérience dans le travail. Son informatisation ne fait que commencer et sa mise en place s’avère comme des bribes dans quelques établissements de santé. Pour le cas du CHU de Sétif, l’informatisation du Dossier Médical Du patient est en cours de réalisation. | |
