Abstract
Medication conciliation (MD) is presented as an approach in improving the security of the
patient's medication management; it prevents or corrects drug iatrogenesis.
In this context, a 3-month prospective study was carried out in the CHU cardiology
department of Sétif. The patients included had to present with acute coronary syndromes
(ACS) and be hospitalized in this department. Reconciliation is carried out on entry and
discharge.
However, its implementation requires a territorial digital platform shared between the
transitions points of the drug circuit and good coordination with the medical team.

والخروج .
أتاح لنا دراسة التوفیق بین الأدویة ابراز كیفیة تحسین إدارة الدواء من خلال عرض وتصحیح الخطأ الدوائي یتطلب تنفیذه
منصة رقمیة إقلیمیة مشتركة بین النقاط الانتقالیة لدائرة الأدویة والتنسیق الجید مع الفریق الطبي
