Abstract

The present study is to investigate the correlation between concentrations of anti-XOR and the rheumatoid factor (IgM-RF) in sera and synovial fluids from the same patients and the severity of joint disease. The titles of anti-XOR (IgG and IgM) and IgM-RF free and complex in patients with rheumatoid arthritis (RA), arthritis and arthrosis were assessed using an ELISA technique. Agglutination test is used to detect the IgM-RF. The Bovine XOR was purified for use as antigen in the preparation of specific antibodies in rabbits and for the detection of free anti-XOR. The antiserum obtained by immunization was rich in anti-XOR with a titer of 102 400 and was used in the detection of immune complexes XOR-anti- XOR (IgG and IgM). By comparing the concentrations of anti-XOR (IgG and IgM) and IgM-RF in sera with those in synovial fluids in all three categories of patients, no significant differences were observed. However, IgM anti-XOR either in free form or complex predominated. In patients with osteoarthritis, the free IgM exceed 16 and 18.7 times IgG in serum and synovial fluid (LS), respectively. While in patients with RA, IgM complexes exceed IgG for 8 times either in serum or in the LS. Unlike IgM, IgG anti-XOR dominated as complexes. The results showed a correlation between the rate of these complexes and severity of joint disease. Indeed, patients with arthritis show serum IgG-XOR higher (p ˂ 0.05) than those of patients affected by arthrosis. Furthermore, patients with RA showed synovial IgM-XOR higher (p ˂ 0.01) compared to those suffering from arthrosis. The detection of IgM-RF by ELISA showed a higher specificity than the agglutination test. Patients with RA with free synovial IgM-RF (94.4 ± 69.2 mcg / ml) significantly higher than those of patients with arthritis (25 ± 18.31 mg / ml) and synovial IgM-RF complex (41.66 ± 2.20 g / ml) were higher than those of arthrosis patients (14.6 ± 2.23 g / ml). Sera and LS of the majority of patients are C-reactive protein positive confirming the existence of inflammation.
