dc.description.sponsorshipAutoimmune hepatitis (AHI) is a chronic liver disease resulting from a disorder of the immune system, where the latter attacks hepatocytes inappropriately, leading to a progressive destruction of the hepatic parenchyma. This pathology involves a complex interaction between genetic factors, including the HLA-DR3 and DR4 alleles, environmental triggers, and a disruption of immune tolerance, mainly due to a deficit in regulatory T lymphocytes (Treg). The liver has a unique immunological microenvironment, characterized by a strong tendency to tolerance in order to avoid excessive reactions to circulating antigens, but this property can also promote the establishment of self-responses immune when the immune balance is broken. The HAI manifests itself in the infiltration of lympho plasmocytes from the liver, the activation of self-active CD4 and CD8 T cells, as well as the presence of specific autoantibodies, allowing to distinguish two main types of the disease according to the serological profile. The clinical spectrum is variable, ranging from asymptomatic forms to fulminant hepatitis, making diagnosis difficult and requiring the integration of wellestablished clinical, biological, immunological and histological criteria. The treatment is mainly based on corticosteroid-based immunosuppression, often associated with azathioprine, to induce remission in the majority of cases. However, relapses are common, and long-term complications such as cirrhosis or hepatocellular carcinoma justify strict monitoring and the need to develop targeted therapi
